With a mission to reduce poverty in its communities, El-Ada Community Action Partnership works to better focus available local, state, private and federal resources to assist low-income individuals and families to acquire useful skills and knowledge, gain access to new opportunities and achieve economic self-sufficiency.
We are those who meet alongside our honored Veterans to support an unprecedented campaign to dramatically increase successful outcomes for Veterans and their families who are homeless or at risk of becoming homeless. The SSVF Program is designed to help Veteran families quickly regain stability in permanent housing after experiencing a housing crisis and/or homelessness.

Who YOU are:
Are you an individual who cares deeply for the Veterans who have served our country and are now seeking VA Healthcare benefits or community healthcare benefits where Veterans are not eligible for VA care? Are you organized to working within case management, and coordinate care to veteran households? The Healthcare Navigator is responsible for providing services that include connecting Veterans to VA healthcare benefits or community healthcare services where Veterans are not eligible for VA care? The Healthcare Navigator provides case management and care coordination to those veteran households enrolling in healthcare. The Healthcare Navigator works closely with the Veteran’s primary care provider and the Veteran’s assigned interdisciplinary treatment team members.
Essential Duties of the Role: The Healthcare Navigator is responsible for and has commensurate authority to perform the duties and responsibilities listed below.
